Apple Valley Village Health Care Center is currently seeking an experienced Registered Nurse (RN) to join our MDS team! Our skilled nursing facility provides care and services for approximately 162 residents.

In this role, you will coordinate and complete the MDS Assessments for our Long Term Care Residents and work on a team with 2 other highly qualified MDS Nurses.

You will report to the DON.

This is a Part-Time , benefit eligible position working weekdays.

MDS Coordinator responsibilities :

• Oversee and complete MDS and care planning.
• Assist Health Plan Nurse with Medicare scheduling, monthly billing, charting, and issuing notices of non-coverage.
• Work in collaboration with Admissions staff, Health Plan Nurse, and Other Department Managers.
• Perform additional tasks as needed.

MDS Coordinator skills and requirements :

• RN license in the state of Minnesota required.
• Minimum of 2+ years experience preferably in LTC.
• Experience with MDS preferred.
• Proficient in computer software.
• Verbal and written communication skills.
• Detail oriented.
