Founded in 1971, Dialysis Clinic, Inc. (DCI) remains the nation’s largest non-profit dialysis provider. With a team of approximately 5,000 talented and caring employees serving across the United States, DCI provides care to nearly 15,000 patients on dialysis and 3,500 patients with chronic kidney disease.
We offer a variety of services including in-center hemodialysis, home hemodialysis, and peritoneal dialysis (PD). Each position within DCI, from billers to administrators to nursing staff, contributes toward the goal of providing excellent patient care.
Our staff has helped DCI achieve the lowest mortality and hospitalization rates among large dialysis providers for the past 13 years in a row.
The Medical Records Clerk will perform clerical duties associated with obtaining, completing, and maintaining patient medical records requests.
Follows established policies and procedures to ensure effective and compliant record management. Manages process end-to-end for requests of medical records;
including accuracy, minimum necessary, and submission standards. Ensures the privacy and security of all patient records.

- Answers and facilitiates requests for medical and billing records from internal and external parties.
- Responsible for retrieving / collecting medical records and information from the EMR system.
- Review accuracy of records prior to release to external parties.
- Handle record invoicing per state and federal laws.
- Maintain accurate logs and information release forms for medical record requests.
- Research lost or missing records / information in accordance with established procedures.
- Set up reviews for external chart reviews, provide training, and address questions.
- Distribute records and reports to departments as requested.
