Position Summary / Essential Duties :
Collaborates with all members of the health care team, patient and family (or significant others) to coordinate timely and efficient assessment of psychosocial and discharge planning needs and formulation of patient and family centered discharge plans.
Serves a resource to others in work-team and department.
1.Provides psychosocial assessment / intervention to assist patients / families (including clinically complex)
Provides age specific care to patients / families in all age groups.
2.Develops, documents and implements a comprehensive plan based on identified psychosocial needs and / or discharge needs.
- Develops plans with attention to individual patient and family goals
- Provides linkage to community resources and UH network services to facilitate optimal care
3.Communicates / collaborates with the work team using a multidisciplinary approach.
4.Provides guidance / consultation to peers / team regarding routine psychosocial issues and / or discharge planning issues.
5.Provides supervision to social work interns if LISW eligible (completion of supervision hours).

QUALIFICATIONS
Qualifications
Position Requirements :
- Two (2) years’ experience in a hospital setting desirable
- Computer skills
- Theoretical and clinical knowledge regarding health care delivery
- Multi-tasks and prioritizes work
- Communicates effectively with persons of various backgrounds
- Demonstrates strong interpersonal skills and excellent communication skills, both written and verbal
- Exhibits strong organizational skills and work ethic
- Demonstrates a professional demeanor when responding to patients / customers
Education Requirements :
Master’s degree from an accredited School of Social Work
License Requirements :
Licensed by the State of Ohio Counselor and Social Work board at the LSW level; attainment of LISW status is encouraged within one year of eligibility
Preferred Qualifications and Miscellaneous Information :
Post-Master’s experience is strongly preferred. Previous work in psychiatry or mental health is also strongly preferred.
This is a locked, in-patient psychiatric unit. Responsibilities include performing assessments, counseling with the patients and families, providing referrals and community resources, discharge planning, scheduling and planning outpatient follow-up.
This position will cover for PTO’s and government requirements.
