Sinai Urban Health Institute (SUHI) is hiring a culturally competent Community Health Worker (CHW) III with previous health education experience to work across SUHI’s health interventions related to asthma, breast health and diabetes and other interventions including its CHW Support Program.
Under the direction of the CHW supervisor, the CHW is responsible for providing and coordinating health education and case management services.
The CHW serves as a bridge between the patient and the medical system by building trusting relationships with community members served by the program(s).
Specifically, the CHW is responsible for community outreach and providing health education in a variety of settings including health fairs, community events, group sessions, and other formats as needed.
The CHW also helps in identifying barriers to accessing quality care, works with individuals to overcome these barriers, and provides referrals.
For asthma, the CHW works closely with program participants in their homes or virtually to provide comprehensive asthma and healthy homes education.
For breast health, they work with women to obtain timely and routine breast health screenings, assisting women with navigating the health care system to obtain screening results and appropriate diagnostic and treatment services as needed.
For diabetes, the CHW helps clients create personalized wellness plans and promotes diabetes self-management. Activities also include program promotion, enrollment and collecting health information from those interested in SUHI’s programs.
The position may coordinate the work of CHW Is and IIs, assist in their training and mentoring, assist the CHW supervisor with research duties, oversee complex case management situations, coordinate outreach activities and facilitate community relationships.
SUHI is seeking an enthusiastic, passionate CHW candidate who has previous experience in community health education and meets other job requirements outlined below.
Position Requirements
- Must have high school diploma or GED; some college or a degree is preferred but not required
- At least 3 years of relevant experience in the areas of community health, health education, management, administration and training (with at least 2 years of direct community health experience)
- Demonstrated leadership and mentorship experience for at least one year
- Knowledge of Chicago’s West Side and / or South Side communities (resident of West Side or South Side communities preferred)
- Passion for and dedication to working with community members
- Punctual, reliable and willing to learn
- Must have a valid driver’s license and own a car
- Proficiency in Microsoft Word, Excel, Access and PowerPoint preferred
