About Us
PACE Baton Rouge (Program of All-Inclusive Care for the Elderly) is a nonprofit health and supportive services program designed to assist seniors who want an alternative to long-term care placement.
PACE is sponsored by the Franciscan Missionaries of Our Lady Health System (FMOLHS)-the parent company of Our Lady of the Lake Regional Medical Center-the Centers.
The mission of PACE Baton Rouge is to enable the aging population to live with dignity in their communities. With PACE's assistance, participants can remain in the secure surroundings of their own homes and communities while providing family members and caregivers with much needed professional guidance, physical relief and emotional support.
Our team works together to develop individual care plans, goals and carefully monitors progress. Our multi-skilled team-consisting of a doctor, registered nurse, social worker, physical therapist, occupational therapist and registered dietitian-works together to develop patient goals, plan care and carefully monitor progress.
Job Summary
The PACE Center Manager, under the general supervision of the Director, has full responsibility for the effective operations of the PACE Center and the coordination and supervision of all participant services.
Job Requirements
- Experience - Five years nursing experience with at least two years supervisory experience and one year caring for the frail elderly.
- Education - Graduate of Accredited RN School
- Special Skills - Able to function well in a team environment. Possess good organizational, communication and supervisory skills.
Able to effectively handle difficult and unusual interpersonal situations.
Responsibilities
Coordination of Participant Care.Coordinates all center activities including care planning and implementation preparation for Interdisciplinary Conferences and daily meetings, family conferences and all related follow-ups.
Coordinates all activities relating to new enrollees.Communicates and coordinates all participant changes to team members.
Participates in the development of the IDT Plan of Care and is a part of the enrollment processes.Participates in the coordination of a 24 hour care delivery and manages and participates in the 24 / 7 On Call Process.
Regulatory StandardsEnsures that established CMS / DHH regulations and standards are met. Understands PACE Regulations and Level 11 Reporting and ensures that PACE staff understands the Regulations.
Responsible for human resource management of all disciplines to include RNs, LPNs, Therapists, Dietary, Pharmacy Staff, Clerical staff, Adult Care Assistants, Recreational Therapy staff, MSW's and Transportation personnel.
Monitors staffing patterns and determines the number, types of staff and supervision required to meet the agency's responsibilities.
Works closely with the Director in the development and on-going monitoring of contract services including selection of providers and on-going evaluation of quality of services.
Responsible for the Participant Advisory Committee as required by standards. Ensures quarterly meetings take place and coordinates the selection of committee members and chairpersons.
Responsible for the coordination of grievances according to standards and meeting the time frames required.
LeadershipServes a liaison between individual program disciplines, other PACE departments and community service providers.
Implements effectual communication between clinical staff, non-clinical staff and medical staff to provide the best of care and services to the Participants.
Participates in the Management team of PACE and effectively communicates any changes, new standards and rationale to the staff.
Participates in the Management of the PACE Budget.
Medical RecordResponsible for knowledge and administration of the electronic medical record and serves as a Super User for the EMR.
