About The Position
Job Title : Physical Therapy Assistant Job Specialty : Rehabilitation Therapy Job Duration : 13 weeks Shift : Monday-Friday, Days Guaranteed Hours : 32 Experience : New graduates welcome;
SNF experience preferred License : State License Required Certifications : COVID-19 test required 72 hours prior to start date with results issued 48 hours before the start date Must-Have : - Prescreen form- Resume and license verification- Compliance with credentialing checklist Job Description : - Ensure prescreen form is completed and attached with submissions- Attach resume and license verification to applications- Candidates will be declined if documentation is not complete- Perform duties as assigned in a supportive and professional manner- Conduct therapy sessions as directed by supervising physical therapists- Follow treatment plans and document patient progress- Maintain a safe and clean working environment- Participate in training and development activities- Complete required background checks and verification processes Credentialing Checklist : State License Verification- Resume- Three references- County Criminal Check (7 years for all counties lived and worked)- Federal Criminal Check (7 years or more)- Multi-State Criminal Search (7 years or more)- Nationwide Sex Offender Search (7 years or more)- HHS / OIG Search- GSA Search- Social Security Number Trace- 1-Step PPD or Quantiferon Test or proof of positive PPD and chest X-ray- SAM Search- Minnesota State Background with Fingerprints
