Keystone Human Services is currently seeking a Supported Living Specialist to join our team of professionals in making a positive difference in the lives of our neighbors and the community.
The Work :
- Develop, oversee and modify the initial, annual and all updates to the assessment for each individual, so that each individual has a current accurate assessment of skills and needs
- Facilitate the development, implementation and on-going follow along of a Person-centered Plan for each individual supported.
- In collaboration with an individual’s team, develop annual Individual Support Plans that meet the specific needs and desires of each individual, and monitor implementation and content of the plans
- Monitor and ensure the consistency and regulatory compliance of all clinical documents
- Develop, educate staff, and monitor implementation of Behavioral Support Plans, plans to address Social, Emotional, and Environmental Needs (SEEN) and Crisis Management Plans that address the unique support needs of each individual using Supported Living services
- Support the individual’s team around issues, such as, but not limited to : behavioral incidents, psychiatric hospitalizations, coaching through difficult situations, crisis management and skill training
- Provide the assistance, education, and support to employees that enable them to provide habilitation services and teach people the skills needed for individuals to remain independent and self-sufficient in the community
- In collaboration with an individual’s team, develop plans to assist individuals to find safe, affordable housing, develop and maintain landlord relationships, develop and maintain a budget and manage finances to support living in the community, utilize generic resources, increase opportunities to develop friendships, maintain and strengthen family relationships, increase participation in desired community activities, and find and maintain meaningful employment
- Participate in the continued commitment to quality improvement
- Actively participate in agency, department and program activities
- Develop and maintain excellent, collaborative, and cooperative relationships with all management staff as appropriate
- Participate in educational activities that will enhance professional and personal development
- Maintain an effective, responsible, and productive working relationship with supervisors, peers and other employees
- Ensure that client confidentiality is maintained at all times
- Consistently communicate and practice the agency’s mission, guiding values, and principles
- Implement Keystone’s policies and procedures and all state and federal regulations pertaining to the services provided
- Perform other duties as assigned

Minimum Educational Requirements : (One of the following)
- Bachelor’s degree and two years of experience working directly with persons with intellectual disabilities or autism
- Master’s degree or higher and one year of experience working directly with persons with intellectual disabilities or autism
Additional Minimum Requirements :
- At least 18 years of age
- Basic computer skills with the ability to utilize internal software programs
- Effective communication skills, both verbal and written
- Successful completion of required trainings within specified timeframes, including First Aid and CPR certifications and others mandated by regulations
- Valid Driver’s License with daily access to a privately maintained and insured vehicle
- Successful completion of the pre-employment process, including motor vehicle record and criminal background checks
