We invite you to join our newest team at the Summit at Plantsville Center for Health & Rehabilitation!-Full time : 40 hours every other weekend 11-7pmPart time : 24 hours every other weekend 11-7pmWhat You'll Do : As a RN Admissions Nurse, you will play a pivotal role in the admission process, utilizing your nursing expertise to assess, plan, and coordinate the transition of residents into our facility.

Your compassionate care will ensure a smooth and comfortable entry for those in need of our services.Key Responsibilities : Conduct comprehensive nursing assessments for potential residentsCollaborate with healthcare teams to review medical histories and care needsAssist in developing individualized care plans for new admissionsCoordinate and communicate care plans with residents, families, and healthcare professionalsEducate residents and families about facility services, procedures, and care expectationsEnsure compliance with regulatory requirements during the admission processIf you are a compassionate and experienced nurse with a talent for guiding individuals through healthcare transitions, join our team as an Admissions Nurse.
