- Responsibilities : * Provide health education to clients in individual and group setting* Monitor prescriber scheduling with specific attention to provider productivity* Review of lab work and coordination with prescriber, as necessary* Documentation of all nursing activities provided* Support clients through nursing case management supports* Support prescribers with clerical support* Administer medications to include inject-able medications while maintaining precise medication records* Provision of emergency medical care to clients until emergency services arrive, when necessary* Coordination of client care with internal and external providers* Ability to work independently and as part of a team* Other duties as assignedBenefits : * Competitive salary* Supplemental income* Medical, Dental, Vision, 403(b)* Generous paid time off benefits* Opportunity for personal and career growth* Team oriented environment - we practice the FISH!
