What You'll Do :
As a Hospice Chaplain, you will carry out our Mission of Love by providing spiritual care support to any situation and need that arises with primary focus on hospice patients, family members and associated caregivers.
This support occurs through the life range continuum of pre-hospice conditions to bereavement follow up.
Our Hospice Chaplain is responsible for the following :
- Promote the role and identify of faith, spirituality, and chaplaincy as an integral part of the hospice experience.
- Demonstrate ability to meet individuals where they are at in the journey regardless of their cultural or spiritual beliefs.
- Conduct spiritual assessments on all patients and families at admission and on-going. Provide spiritual care support on a routine and referral basis.
- Refer to appropriate resources as needed.
- Provide psycho-emotional-spiritual support through discussion of end-of-life issues with patients, families, and hospice team members.
- Demonstrate proficiency in skills of active listening, life review, validation, compassion, God conversation and servant leadership.
- Create and lead meaningful funeral and remembrance services as requested.
- Develop and lead educational trainings and in-services on spirituality and end of life topics for staff, referral sources and the community at large.
- Participate in team meetings and other committees as assigned.
- Participate in the supportive services on-call rotation (casual employees are exempt from this requirement).
- Demonstrate proficiency in explaining the Medicare Hospice benefit, how hospice can help, and how spirituality and faith are a part of the end-of-life process.
- Present oneself in a substantive manner indicative of spiritual awareness.
- Routinely collaborate with team members, referral sources and facility staff to ensure that spiritual needs are being met.
- Facilitates opportunities for all to engage in spiritual and ethical discussions regarding end of life.
- Carry out QAPI roles and responsibilities as assigned.
What You Have :
- Bachelor's degree in Ministry, Theology or related field preferred.
- Masters degree or CPE beneficial, but not required.
- Maintain a current PA driver's license and insurance on personal vehicle.
- Personal Computer experience required.
