Job Title : Hospice Intake Coordinator and DME Orders
Job Summary :
We are seeking a highly organized and compassionate Hospice Intake Coordinator and DME Orders to join our team. The ideal candidate will be responsible for coordinating the intake process for hospice patients and managing the ordering and delivery of durable medical equipment (DME) for our patients.
The Hospice Intake Coordinator and DME Orders will work closely with the hospice team to ensure that patients receive the highest quality of care.
Responsibilities :
- Coordinate the intake process for hospice patients, including obtaining necessary medical records and insurance information
- Communicate with patients and their families to provide information about hospice services and answer any questions they may have
- Work with the hospice team to develop and implement care plans for patients
- Manage the ordering and delivery of DME for patients, including coordinating with vendors and ensuring timely delivery
- Maintain accurate and up-to-date records of patient information and DME orders
- Collaborate with other healthcare professionals to ensure that patients receive comprehensive care
- Provide support and guidance to patients and their families throughout the hospice care process
Requirements :
- High school diploma or equivalent; associate's or bachelor's degree in healthcare or related field preferred
- Minimum of 2 years of experience in hospice or healthcare coordination
- Knowledge of hospice regulations and guidelines
- Excellent communication and interpersonal skills
- Strong organizational and time management skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office and electronic medical records (EMR) systems
- Compassionate and empathetic demeanor
