supervision and guidance of Assistants and consultation with referral sources and other healthcare providers.

Requirements Qualifications

• Licensed or eligible for licensure by the State of Illinois under the Illinois Physical Therapy Practice Act.
• Member or eligible for membership in the APTA preferred.
• General knowledge and therapy skill to perform the evaluation and treatment of a wide variety of conditions; and ages from neonates, pediatrics, young adults, and adults to geriatrics.
• Current certification in CPR and other service line specific competencies.

Essential Job Functions

• Perform examinations and evaluations; establish a Physical Therapy diagnosis and treatment plan.
• Provide evidenced-based Physical Therapy treatment.
• Documentation, reporting, and information dissemination.
• Case management functions : evaluate effectiveness, revise the plan, and discharge from services.
