Job Description

General Description

Provides assistance to people with developmental disabilities to make meaningful connections in the community by helping people supported form relationships with community members and places.

This role works collaboratively with other staff to develop community connections for people and is responsible to : promote a deeper understanding of people's contributions and skills by building relationships with community members and community places;

work to widen the circle of people in the greater community; support a positive vision for community building amongst all teams with a goal of creating greater life enhancement by lessening reliance on paid support.

Essential Functions are minimum requirements of the position :

  • Demonstrates leadership skills including supervision, coaching and motivation for team members.
  • Models and upholds the Five Valued Experiences : Belonging, Respect, Sharing spaces, Contribution and Choice.
  • Promotes people developing and expanding participation in community places and building relationships with community members, family, friends, and neighbors.
  • Pursues a variety of strategies including, but not limited to : connecting people with businesses, and organizations, including religious institutions;
  • developing volunteer opportunities in the community; networking with friends, family, neighbors, associates and community members;

interviewing community members and organizations to develop community building strategies and opportunities.

  • Innovate and experiment to broaden and deepen the range of community building opportunities available to people supported by West Bay.
  • Organize and participate in individualized planning processes to define a community building focus for people supported.
  • Collaborate, organize and participate in presentations, orientations, and periodic meetings about community building activities to assist agency teams to understand the role of community building and its necessity in the lives of people supported.
  • Work with agency staff to develop strategies for maintaining accountability for community building, including : assure community building plans are developed and implemented, assess whether staff actions are leading to meaningful relationships and activities for people supported.
  • Promote appropriate, respectful communication and interactions consistent with agency core values with the supported person in the home and community.
  • Follow all plans of care, report concerns to nurse or management, while attending to the individual's well-being.
  • Maintain a positive, productive team environment with all relationships including individuals, families, management and employees of the agency, as well as community associations.
  • Ability to objectively monitor and accurately document the status of the individuals supported.

Knowledge, Skills and Abilities

  • Extensive knowledge about the capabilities and needs of people with IDD;
  • Ability to effectively communicate across all teams verbally and in writing;
  • Demonstrated computer proficiency including Word, Excel, Outlook, PowerPoint, databases and internet.
  • Capacity to organize and facilitate planning processes;
  • Aptitude to work effectively both independently and collaboratively with agency staff and teams;
  • Ability to provide leadership within the agency in the area of community building;
  • Ability to effectively network;
  • Aptitude to connect with a variety of people in a variety of settings;
  • Skilled in establishing meaningful relationships with people receiving services, families, agency staff and community members that are based on mutual respect and recognition of each person's ability to contribute.

General Requirements

  • Bachelors or Associates Degree preferred in Human Services, Therapeutic Recreation or a related field.
  • Minimum of two to three years' experience supporting people Intellectual / Developmental Disabilities or related population(s) including individuals with high behavioral needs, autism and seniors.
  • Maintain a valid Driver's License; vehicle inspection, registration and insurance.
