Job Title : Advanced Practice Clinician (Nurse Practitioner / Physician Assistant)
Job Overview : One of our largest clients is looking for an Advanced Practice Clinician (APC) to join their team. We are looking for a provider to join their integrated care delivery program that coordinates clinical care for members in a facility setting.
As an APC, you’ll provide care to our highest-risk health plan members as part of an interdisciplinary team.

Work Schedule : Monday to Friday, 8am to 5pm
Primary Responsibilities :
- Conduct comprehensive assessments.
- Manage acute and chronic medical and behavioral conditions effectively.
- Ensure accurate and complete documentation, including ICD 10 conditions.
- Collaborate and communicate with the interdisciplinary care team.
- Conduct advanced illness and advanced care planning conversations.
- Provide counseling and education to patients and caregivers.
Required Qualifications :
- Active and unrestricted license in the state of residence, or ability to obtain by start date.
- Education :
- NP : Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
- PA : Graduate of an accredited Physician Assistant degree program.
- Active Nurse Practitioner or Physician Assistant certification through a national board.
- Current, active DEA licensure / prescriptive authority or ability to obtain post-hire, per state regulations.
- Access to reliable transportation
- Ability to gain a collaborative practice agreement.
Preferred Qualifications :
- 1+ year of clinical experience in practice (long-term care setting preferred).
- Experience in meeting the medical needs of patients with complex behavioral, social and / or functional needs.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end of life discussions.
- Proficient computer skills including the ability to document medical information with written and electronic medical records.
