Who We Are : Since 1985, BHCHP’s mission has been to ensure unconditionally equitable and dignified access to the highest quality health care for all individuals and families experiencing homelessness in greater Boston.

Over 12,000 homeless individuals are cared for by Boston Health Care for the Homeless Program each year. We are committed to ensuring that every one of these individuals has access to comprehensive health care, from preventative dental care to cancer treatment.

Our clinicians, case managers, and behavioral health professionals work in more than 60 locations to serve some of our community’s most vulnerable and most resilient citizens.

Job Summary : As a full-time case manager, you will be primarily based in the JYP / BMC Clinic where you will work with one of the primary care teams based at this site.

You will provide as-needed case management support for the primary care team, specifically focusing on patients who are not eligible for BHCHP care management programs.

Case management services include, but are not limited to benefits (SSI, EAEDC), housing and legal support, connection to primary care and behavioral health.

This position offers an excellent growth opportunity for an individual with case management experience, flexibility, and interest in working within Boston Health Care for the Homeless’ primary care teams.

  • Hours : Monday-Friday 8 : 00am-4 : 30pmResponsibilities : Work in our main clinic with a multidisciplinary team of providers, nurses, behavioral health clinicians, and case managers;
  • collaborate with medical providers; identify and develop cooperative working relationships with service providersProvide case management support to a small BHCHP primary care panelProvide a blend of scheduled clinic sessions seeing walk-in patients, as well as flexible case management support for teams across the programParticipate in city-wide networks of case management for homeless individualsQualifications : Able to work with multidisciplinary team maintaining a good rapport with nursing staff, medical staff, other departments, and visitors;
  • self-directed with the ability to work independently in multiple settings; flexible and adaptable to different health care delivery modelsStrong problem solving and communication skills (written and oral);

efficient, organized, detail-oriented, and able to complete tasks in a time-sensitive manner; excellent customer service skills and the ability to communicate professionally with employees and patients, both on the phone and in personThe compensation starts at $22.

07 per hour and increases with years of experienceKnowledge of the network of services available to homeless persons, and experience working with homeless persons preferred, prior case management experience a plusComputer skills : proficient with Microsoft Office, entering narrative and other data into electronic medical records and other internet-based productsSpanish or Haitian Creole language skills preferredCompensation : The compensation starts at $22.

07 per hour and increases based on years of experience.BHCHP full time employees are eligible for our competitive time off policy of 4 weeks’ vacation, health, dental and vision insurance, 403B retirement savings plan and employer retirement contribution, and pre-tax MBTA pass program with 40% discount.

In addition, eligible employees will receive yearly increases, additional compensation of seven thousand five hundred dollars added to your base hiring rate for demonstrated bilingual proficiency and the opportunity to work with local hospitals and community health centers.
