The MDS Coordinator - Registered Nurse plays a vital role in conducting accurate and timely Minimum Data Set (MDS) assessments, collaborating with interdisciplinary teams to enhance resident care plans, and ensuring compliance with regulatory requirements.
Responsibilities include coordinating MDS assessment completion, developing individualized care plans, scheduling care conferences, monitoring resident condition changes, processing Medicare documentation, and providing staff education on MDS / PRI and care planning.
- Three (3) years of clinical experience in acute or long term care setting.
- Strong understanding of MDS processes, regulations, and reimbursement systems (RUGS and PDPM).
- Excellent communication and interpersonal skills.
- Proficiency with MS Office, databases, and medical software.
