Community Healthcare Network is seeking for a Full-Time Behavioral Health Therapist who will be an integral part of the Behavioral Health treatment team providing clinical treatment services under the direction of the Behavioral Health Director and treating Psychiatrist.
JOB FUNCTION :
Responsibilities include, but are not limited to :
- Provides assessment and screening services to individuals interested in becoming participants in the mental health program.
- Conducts and evaluates intake and psycho-social assessments and makes diagnoses.
- Provides treatment services to a caseload of 30-50 clients, the caseload number being based on the following factors : show rate, modalities provided (i.
e. individual therapy only, group and individual therapy) and maintaining weekly utilization / billing targets.
- Develops and implements treatment plans according to OMH standards.
- Provides psychotherapy and psycho-education to a variety of clients, utilizing individual, family and group models.
- Involves clients in their own treatment planning as well as their own education about their mental illness(es)
- Develops and maintains clinical records, in accordance to OMH standards.
- Participates in case conferences with case management staff and other members of the clinical team.
- Meets with consulting psychiatrist in the development of the treatment plan and to obtain direction.
- Provides cases intervention, which could include home visits, escorting clients to emergency room, assisting in the development of plan for clients at risk for suicide / homicide.
- Performs other related duties, as assigned.

- Licensed Master Social Worker
