Lighthouse Behavioral Health Solutions (LBHS) provides individual, outpatient, intensive outpatient, partial hospitalization, residential, psychiatric services, and medication assisted treatment for mental health and substance abuse treatment needs.

Position : Care Coordinator

Job Summary : Supporting individuals dealing with substance abuse or addiction issues as they transition into sober

living environments.

Duties and Responsibilities :

  • Provide ongoing case management services to residents, including regular check-ins, goal setting, and progress monitoring.
  • Serve as a liaison between the recovery housing program and treatment providers, facilitating communication and information sharing regarding case management services.
  • Coordinate and schedule appointments with treatment providers, ensuring residents have access to necessary services and follow-up care.
  • Assist residents in accessing community resources and support services, such as employment assistance, educational programs, and legal services.
  • Advocate for residents' needs and rights, ensuring they receive appropriate and quality care.
  • Conduct regular meetings with residents to address common challenges, provide education, and promote community-building.
  • Collaborate with the recovery housing program staff to ensure a safe and supportive living environment for residents.

Maintain accurate and up-to-date documentation of resident interactions, progress, and treatment plans.

Participate in team meetings and treatment team meetings to discuss resident progress and develop strategies for improved care.

Provide crisis intervention and support to residents in times of relapse or other emergencies.

Conduct medication counts at the request of clinical staff.

Coordinate discharge planning and support residents in transitioning to independent living or other appropriate levels of care.

Collaborate with external agencies, community organizations, and treatment providers to enhance the resources available to residents.

Required Qualifications :

  • Minimum high school diploma / E.D
  • Valid CDCA
  • Knowledge of Electronic Health Records systems
  • Valid State of Ohio Driver's License and insurance
  • Must pass BCI check, all Corporate Compliance checks, and employment drug
  • Familiarity with computers and technology / computer programs (Word, Excel, Outlook and Teams)
  • Must be 25 years or older

Schedule :

Tuesday - Saturday 7am - 3pm

hours and days may vary depending on business need)
