Job Summary
The Ex tended I ntervention for T obacco Use (EXIT) study is aimed at increasing access to cessation services and reducing tobacco use among people experiencing homelessness.
The EXIT study intervention includes : 1) community pharmacist-linked consultation with 6-month delivery of medications for cessation, and 2) health coach-delivered weekly to monthly wellness-focused coaching addressing tobacco use within the context of co-occurring psychiatric and substance-use diseases and other life stressors.
A key role for health coaches is to motivate and engage clients to set and achieve their goals through weekly-to-monthly telehealth sessions.
EXIT Health Coaches will use a process-oriented coaching approach by asking participants to : first, define a vision using a self-assessment form that focuses on goals for tobacco cessation, mental health, substance use, exercise / physical activity, and food / nutrition, and then identify specific, measurable, achievable, relevant and time-bound (SMART) goals to achieve that vision;
second, take steps to reinforce and maintain that vision through empowerment, skill-building, participant-elicited education, and building self-efficacy through self-assessments and scaling questions (e.
g., level of confidence on a scale of 1 to 10); and third, address roadblocks to behavior change by eliciting information on triggers, strategies to address those roadblocks.
Health coaches will participate in skills-based training through weekly learning collaboratives, structured tobacco treatment specialists training, and coaching training with the study team.
Health coaches will serve as navigators to facilitate interactions between participants and pharmacists and their primary care providers (PCPs) or local health centers if they don't have PCPs around receipt of medications, side effects, dose escalations, or medication changes.
Ideally, the candidate will have lived experience of homelessness, housing insecurity, and tobacco smoking in themselves or their relatives.
NOTE : This is a 25% 2-year Contract role.
The final salary and offer components are subject to additional approvals based on UC policy.
Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF.
For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.
The salary range for this position is $50,700 - $107,500 (Annual Rate).

Department Description
The Department of Medicine (DOM) is the largest department in the School of Medicine (SOM) here at UCSF, with more than 600 full-time faculty.
The mission of the Department includes research, teaching, patient care, and public service. The Department has a decentralized organizational structure across four locations (Mission Bay, Parnassus Heights, San Francisco Veterans Affairs Health Care System, and the Zuckerberg San Francisco General) and approximately 38 divisions.
UCSF DOM has consistently ranked among the top four departments of medicine in the country. Authority and accountability of the Department rests with the Department Chair (leads Parnassus Heights site) and directs the Vice Chairs / Chiefs at the SFVAHCS and ZSFG.

Required Qualifications
- Bachelor's degree in related area and / or equivalent experience / training.
- Experience and advocacy working with people experiencing homelessness, individuals with mental health and substance use disorders, and with person using tobacco.
- Basic knowledge of the health promotion and education field.
- Skills associated with the delivery of health promotion and education programs.
- Effective interpersonal skills needed to work with a diverse client group.
- Ability to communicate effectively both verbally and in writing.
Preferred Qualifications
- Basic technological skills (e.g. Zoom, smartphone applications, health dashboards).
- Familiarity with health coaching, participant navigators and patient / family-centered healthcare.
- Coaching and motivational interviewing skills.
- Basic knowledge of health conditions related to tobacco use.
- Familiarity with SMART goal setting.
- Strong organizational skills and attention to detail.
- Ability to work independently and manage multiple participants simultaneously.
- Experience with mindfulness meditation.
- Experience with cognitive-behavioral therapy.
- Experience with exercise, yoga, Tai Chi, or other movement programs.
