About The Position
Weekly Pay Range - $1879 To $2087 Job Title : Care Manager Job Specialty : Utilization Management Job Duration : 9 months Shift : Hybrid 9-5 Guaranteed Hours : 40 hours per week Experience : Minimum of five years of clinical experience as a Registered Nurse;
leadership experience preferred. License : Registered Nurse, Licensed Practical Nurse, or Physical Therapist license required.
Certifications : Baccalaureate degree required; PRI and Screen certification within three months of employment; Master's Degree and Bilingual Spanish Certification in Case Management preferred.
Must-Have : Experience in Care Coordination, Utilization Review, and Discharge Planning- Computer literacy- UM / UR experience in managed care or hospital setting Job Description : Provide care management and coordination services.
- Conduct utilization reviews and plan discharges.- Collaborate with healthcare teams to ensure appropriate care and resource allocation.
- Demonstrate strong leadership and communication skills.- Maintain and document patient care activities in compliance with regulatory requirements.
- Utilize skills in a hybrid work setting, balancing in-office and remote responsibilities.- Engage in continuous learning and certification to meet industry standards.
