Job Summary
Aptive Resources, in collaboration with the Fayetteville VA Medical Center, is seeking a highly skilled and compassionate Primary Care Nurse Practitioner to provide comprehensive family practice consultative services to Veterans in an outpatient setting.
As a member of the Primary Care team, you will play a vital role in assessing, diagnosing, and managing the healthcare needs of our nation's heroes.
Schedule : Monday through Friday 8am-5pm
Primary Responsibilities
- Conduct thorough patient assessments, including health history, physical examination, and diagnostic evaluations.
- Provide patient education on disease prevention, treatment options, and medication management.
- Order and interpret diagnostic tests as necessary, and collaborate with physicians to develop appropriate treatment plans or referrals.
- Record and document all patient encounters accurately and in a timely manner using the electronic comprehensive patient record system (CPRS).
- Develop and implement individualized care plans based on patient needs and preferences.
- Monitor patient progress, adjust treatment plans as needed, and ensure continuity of care.
- Collaborate with interdisciplinary team members to coordinate care and facilitate communication between primary care providers and specialty services.
- Adhere to VA policies, procedures, and standards of care to ensure compliance and quality of service delivery.
Minimum Qualifications
- Master's degree in Nursing from an accredited program.
- Current, unencumbered license to practice as a Nurse Practitioner in a State, Territory, or Commonwealth of the United States or the District of Columbia.
- United States citizenship.
- Certification in VHA-approved Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) from the American Heart Association.
- Certification as a Family Nurse Practitioner by the American Nurses Credentialing Center (ANCC).
- Minimum of 5 years of experience as a Nurse Practitioner in family practice within the last five years.
