Job Title : Records & Information Clerk

Job Locations : City of Norfolk, Norfolk Community Services Board

Schedule : Monday Friday 8 : 30am-5 : 00pm

Pay Rate : $19.56 hourly .

Job Overview :

Responsibilities include the creation, maintenance and safekeeping of all client’s medical records. Converting physical documents into computer-based records.

Serves as a resource person to staff regarding electronic health information. Process all requests for medical record information for our consumers and external agencies.

Job Duties :

• Observe confidentiality Laws and Regulations and safeguards all consumer protected health information.
• Responsible for the coordinating and processing of release of medical record information.
• Verify authorizations in accordance with Department policies and procedures and state and federal laws.
• Ensure that all requests for records are stamped with date received and logged in the Diklosure Log.
• Daily scan and attach paper documents into the consumers' electronic health record.
• Collecting patient information and assisting in audits.
• Process closed paper records, identifies and prepares physical records for appropriate storage location.
• Retrieving, compiling, and storing paper medical records.
• Adhere to Norfolk CSB and the City of Norfolk policies and procedures.
• Provide excellent customer service.

Experience :

• Ability to multitask.
• Detail oriented
• Ability to handle difficult situations involving consumers, staff, and / or other in a professional manner.
• Knowledge of medical records departmental functions.
• Knowledge of state and federal laws regulating patient rights and privacy.

Physical Requirements :

Requires walking, lifting heavy records in boxes, bending, reaching, stooping, and stretching. Requires sitting for approximately 4-6 hours per day.

Requires eye hand coordination and manual dexterity.

Requirements :

High School Diploma / GED and one year of medical records experience. Strong interpersonal skills and ability to communicate.

Computer experience required.
