Key ResponsibilitiesConduct comprehensive and symptom management visits to create and oversee individualized care plansOrder and interpret diagnostic tests, prescribe treatment modalities, and facilitate patient transitions within healthcare settingsCollaborate with multidisciplinary teams and provide educational tools to patients, families, and staffRequired Qualifications and EducationCurrent, unrestricted RN and NP licenses in California and OregonDEA licensure or the ability to obtain itGraduate of an accredited nurse practitioner programANP, AGNP, or FNP Certification;
ACHPN certification preferred2+ years of experience providing NP care to complex, seriously ill patients
