Soliant Health is seeking a flexible and experienced Travel Pharmacist to work throughout Minnesota at various locations.
This opportunity is perfect for someone looking for full-time hours across multiple sites in Minnesota. The ideal candidate will have long-term care pharmacy experience and be willing to travel across these various locations.
You will be providing essential pharmacy services in a specialty pharmacy setting, ensuring high-quality care for patients in need of medication management and long-term care pharmacy support.
Key Responsibilities :
- Medication Dispensing : Prepare and dispense medications for long-term care patients, ensuring safety, efficacy, and pliance with regulations.
- Clinical Consultations : Provide clinical consultations regarding medication therapy management (MTM) for patients and collaborate with physicians and other healthcare providers to optimize medication use.
- Medication Therapy Management (MTM) : Utilize MTM skills to assess patient medication regimens and provide remendations to improve outes, prevent drug interactions, and enhance overall therapy.
- Patient Counseling : Educate patients on their medication regimens, including possible side effects, proper usage, and the importance of pliance.
- Inventory Management : Manage and monitor drug inventories across multiple locations to ensure proper medication supply is maintained.
Work closely with pharmacy technicians to ensure accuracy in preparation and delivery of medications.
pliance and Safety : Adhere to federal and state pharmacy regulations and maintain records of drug dispensing, inventory, and other critical data in pliance with the pharmacys policies and procedures.
Ideal Candidate Qualifications :
- Licensure : Must hold an active and unrestricted Minnesota pharmacist license at the time of application.
- Education : PharmD or MTM certification is required. Candidates with a strong background in long-term care pharmacy are highly preferred.
- Experience : Prior experience in long-term care pharmacy settings is essential. Expertise in medication therapy management (MTM) is a plus.
- Travel Readiness : Must be willing to travel to the different sites listed, with flexibility in location and scheduling.
- Technology Proficiency : Familiarity with pharmacy software systems, including inventory management and patient record systems.
