Role Overview :
Join our client as a Patient Care Coordinator, where you will utilize your clinical and administrative skills to ensure high-quality patient care.
The ideal candidate is a Registered Nurse with at least two years of pediatric experience.
Compensation : $100,000.00
Location : Long Island City, New York
Schedule : Monday to Friday, 8 am - 4p m or 9 am-5 pm
Responsibilities :
149; Anticipating patient needs and ensuring completion of pre-visit planning
149; Reviewing daily patient appointments and preparing, distributing, and analyzing daily patient rosters
149; Coordinating and documenting daily patient care conferences
149; Providing proactive outreach to high-risk patients and facilitating advanced management of their care
149; Developing individualized care plans by collaborating with providers and assisting patients in setting treatment goals
149; Educating patients on their care plans and properly documenting interactions in eCW
149; Participating in QI Team meetings and collaborating with Benefits Counselors, Outreach staff, and the HIM supervisor to ensure appropriate patient scheduling, flow, and documentation procedures
149; Performing other duties, assignments, or special projects as needed
Qualifications :
149; Valid New York State RN License
149; Minimum of 2 years of pediatric experience
149; Prior clinical experience preferred; administrative experience also preferred
