Summary :
The ideal LPN candidate will oversee coordinate anddirectly address healthrelated concerns of program participants.This role requires a strong sense of responsibility dedication topatient care and the ability to travel to different locationsthroughout their shift.
EssentialDuties and Functions :
- Maintain accurate and current nursingdocumentation and medication administrationrecords.
- Educate and assist residents withmedication management and adherence to physicianorders.
- Provide various nursing care servicesincluding but not limited to wound care trach care and diabeticcounseling.
- Assess the health and emotionalneeds of residents and adjust care plansaccordingly.
- Communicate efficiently with thenursing team program staff and management regarding residentconcerns.
- Uphold the highest level ofconfidentiality in all aspects of the job.
CredentialsRequired :
- ValidLicensed Practical Nurse (LPN)license.
- Pennsylvania Criminal Clearance(obtained within the past year).
- Current CPRcertification.
- Proof of Covid19Vaccination.
- Valid driverslicense.
- Recent Physical and TB or Quantiferontest (within the last 6 months).

Qualifications :
- Demonstrated experience as an LPN preferably ina residential or community healthsetting.
- Exceptional assessment organizationand problemsolving skills.
- Ability to operateindependently and as part of a multidisciplinaryteam.
- Effective communication skills with astrong emphasis on confidentiality.
- Mustpossess reliable transportation for travel between residentiallocations.
