Responsibilities :
- Supervise care and treatments, lead patient assessments, create personalized care plans and resolve patient concerns.
- Build relationships with patients and their families, learn about their needs to help them recover.
- Communicate and collaborate with other departments to coordinate care and assist with patient needs.
- Be a resource for the staff and celebrate patient wins along the way.
Requirements :
- Current licensure or certification required by state regulations.
- CPR certification required (ACLS preferred).
- CRRN certification to be obtained within a year of meeting the eligibility requirements.
- At least one year of experience in a rehabilitation hospital setting (within the last five years)
