Job Description
Facilities clinical admissions to inpatient acute / sub-acute rehabilitation care. Serves as a liaison for referral sources and promotes available rehabilitation services.
Communicates required information to the inpatient clinical team.
Job Responsibility
1.Receives referrals for rehabilitation care from internal / external inpatient / outpatient facilities.
2.Performs clinical screening for admission based on regulatory, department and insurance guidelines. Reviews necessary medical records and may visit referring facilities of patients to be admitted.
3.Identifies, communicates and coordinates for any special needs of patients during admissions process.
4.Collaborates with required stakeholders (ie. physicians, clinical professionals, administrative and clinical support team members) to facilitate admissions process.
5.Completes and processes required admissions documentation.
6.Establishes and develops community / physician referrals for possible admissions.

Job Qualification
Graduate from an accredited clinical education program, required.
Current related Clinical License to practice in New York State, required. (RN, or LPN)
Current NYS Driver's License, required.
1-3 years of relevant experience, required.
Strong communication and customer relation management skills.
