Primary Aide positions available at Omni, our residential treatment program for men (www.socialmodel.com).. Some positions are GRAVEYARD shifts.
The Primary Aide's focus is to assist Primary Counselors in providing services to participants. However, this position may be asked to perform the core functions of a Counselor as assigned and when needed.
Functions may include but are not limited to screening, intake, assessment, orientation, individual counseling, treatment planning, case management, and documentation.
- Role model appropriate healthy and ethical behaviors in all interactions, including HIPAA and Confidentiality standards.
- Monitor all residents on an hourly basis, ensuring their safety. If working on a night shift, a Night Rounds Checklist should be completed, also on an hourly basis.
This form will indicate the current physical status (whereabouts) of the resident.
- If working a morning shift, ensure residents are woken up at the appropriate time.
- Administer resident drug tests when directed.
- Provide case management services as needed, including documentation.
- Facilitate full utilization of service units, including case management documentation, as directed by the supervisor.
- Document the services provided according to standards set by the Agency and applicable County, State, and Federal regulations.
Documentation should be clear, concise, and completed in a timely manner.
- Assist with resident’s benefits (e.g. DPSS, SSI, Medi-Cal) application(s).
- Monitor resident medications, as applicable, including documentation.
- If directed, inventory new resident property upon admission.
- If directed, upload appropriate documentation into resident’s electronic health record.
- Familiarity and adherence to all regulatory expectations and ethical standards and Agency policies and procedures, including the Agency’s Core Beliefs.
- Maintain constructive relationships with family members, community members and organizations, human services agencies, and other professionals.
- Assist in ensuring a safe and alcohol / drug-free environment.
- Perform all other duties as assigned.
Credential required (Salary commensurate with experience and credential) : SUD registered or certified.
Non-smoker preferred.
