The Role

Under general direction, the Addiction Counselor is an integral member of the clinical team, serving as the primary counselor to a caseload of patients with substance use disorders. The Addiction Counselor will manage a caseload, provide individual and group counseling, conduct assessments, coordinate services, psycho-education, referrals, and keep all documentation up to date.

(Addiction Counselor Assistant) Additionally, the Addiction Counselor Assistant will provide administrative and case management support within residential treatment programs, assisting with sample swabs, screenings, orientation, intake, psycho-education, referrals, reporting, recordkeeping, co-facilitating groups, and basic crisis intervention.

Responsibilities

What You Will Do

• Develop treatment plans in collaboration with the Program Director or Clinical Supervisor, reflecting understanding of specific issues related to physical, medical, cognitive, and psychosocial needs.
• Use a strengths-based and trauma-informed approach to work collaboratively with patients and treatment team members to support recovery.
• Provide clinical consultation to referral sources, mentors, family members, and others to support patient recovery.
• Engage patients in decision-making and help them understand the benefits of assessment and appropriate level of care.
• Review referral information and coordinate services and scheduling of patient interviews.
• Make referrals to suitable programs and agencies based on assessments to meet patients' treatment needs.
• Assist patients with scheduling admissions or first appointments at referred programs or facilities.
• Monitor patient involvement with treatment providers, addressing barriers to care and recovery.
