Job Description :

The Case Manager I plays a crucial role in supporting students enrolled in the Reisher Bridge Scholars program through personalized guidance and assistance.
This position focuses on facilitating students; successful transition from Aims Community College to Reisher Scholars 4-year partner institutions.
This position helps students navigate complex processes such as transfer admissions processes, financial aid, and other scholarship applications, ensuring they make informed decisions aligned with their academic and career goals.
By maintaining accurate records and leveraging resources, this position enhances student support and ensures compliance with institutional policies and grant guidelines.
Additionally, the Case Manager I contributes to the overall success of the Reisher Bridge Scholars program by supporting the coordination of program activities, monitoring progress towards degree completion and program requirements, and providing referrals and interventions when needed.
Through strategic planning and effective communication with stakeholders at Aims and transfer institutions, the Case Manager I plays a pivotal role in achieving program objectives and enhancing student success outcomes.
Note : This position is part of a five-year grant-funded pilot through June 30, 2029. Position renewal after the grant term ends is subject to grant renewal and funding availability.
Job Duties :
Reisher Bridge Scholars Program Case Management Lead :
- Offer 1-1 personalized guidance and support to students navigating the requirement of the Reisher Bridge Scholars Program and navigating the transfer, Reisher Scholars, financial aid, and scholarship application process at their transfer institution.
- Provide guidance in the selection of transfer school(s) aligned with the student’s academic, personal, and professional needs and goals.
- Help students understand the difference between transfer institutions, including program offerings, campus culture, and career outcomes.
- Identify and leverage relevant transfer-related resources and tools to support students; transfer journeys.
- Maintain student files, databases, and records in accordance with institutional policies / procedures and federal guidelines (FERPA).
- Ensure data accuracy and update records regularly for institutional reporting, grant requirements, and tracking student success.
- Offer immediate assistance and referrals to students facing crises, ensuring their safety and well-being in accordance with institutional policies.
- Monitors academic progress of students toward degree completion, identifies current and potential needs or problem areas (e.
g. study skills, tutoring) and refers students to appropriate campus resources for assistance.
Checks various program-related and transfer / RSP specific forms for accuracy, assists students in their completion, signs related forms when appropriate, and documents student interactions in appropriate tracking systems.
RBSP Program Delivery & Support :
- Support the management of day-to-day operations of the RBSP to ensure grant goals, objectives, and KPIs are met annually.
- Help coordinate, implement, and assess all Reisher Bridge Scholars program activities, including scheduling, logistics, and event execution (e.
g., informational meetings, orientation session, community building events, workshops, and final transition event).
- Support the facilitation of a smooth transition for students, increasing their financial preparedness and familiarizing them with their transfer institution.
- Assist in the development, implementation, and assessment of 1-1 transition coaching sessions for program participants.
- Develop and manage the annual program participant communication plan (emails, text, phone calls, newsletters, D2L course shell content, etc.).
- Co-develop, implement, and assess internal processes and procedures that ensure participants are effectively guided through transfer, RSP, financial aid, and scholarship application processes at the transfer institution.
- Maintain and update procedural documentation.
- Assist in implementing a student-level data tracking system to meet all annual reporting requirements of the grant. Input data for reporting purposes and assist in the preparation of required reports and documentation for grant compliance.
- Support the development and maintenance of high-level relationships and effective communication with admissions, RBS program staff, and support resources at both Aims and transfer institutions.
- Assist in developing, implementing, and assessing strategies to foster connections and a sense of belonging for participants, including organizing and assessing events and activities that promote community building.
- Help implement and assess personalized support and educational opportunities for participants to increase their financial preparedness to complete a bachelor’s degree.
- Co-implement and assess opportunities for participants to visit RBS programs, explore campus environments, and become familiar with facilities and resources at their desired transfer institutions.
Coordinate logistics for campus visits, including arranging tours and meetings with key personnel.
- Create, prepare and organize materials for meetings, workshops, and other program-related activities.
- Serve as the primary point of contact for prospective and current participants, addressing their questions and concerns promptly.
Maintain accurate and up-to-date records of participant interactions, program activities, and outcomes.
Minimum Qualifications :
Bachelor’s degree in Psychology, Sociology, Human services, or other related field; Plus,
2 years’ experience in academic advising or personal, academic and / or career coaching or counseling in education or clinical setting or an equivalent combination of education and / or work experience.

This position requires the use of a company vehicle and / or personal vehicle for business travel. The selected candidate must successfully pass an MVR and be insurable under Aims insurance.
