The Patient Care Coordinator will work closely with the Case Managers as well as the therapist and providers to assist in scheduling appointments, providing referrals and the mission and goals of the organization!
ESSENTIAL DUTIES & RESPONSIBILITIES :
- Coordinate and schedule medical and behavioral health appointments for adults and those working with the Prevention Case Manager
- Track and monitor the therapist’s and Prevention Case Manager’s patients.
- Monitor tasks and orders of assigned providers
- Complete a full record review of Adults sent to the behavioral health pool
- Follow the most current work-flow of the Patient Care Coordinator
- Perform related duties necessary to ensure quality patient care as requested by supervisor, including addressing barriers when the patient has not met treatment goals
- Process all tasks and orders, including phone calls from patients, routing as appropriate and responding in a timely manner
- Scan and fax documents to be read, reviewed, placed in appropriate charts and / or routed as appropriate daily
- Coordinate, track and monitor outside referrals (e.g. Direct Secure Message, obtain outside specialty records)
- Facilitate linkage to services to mitigate Social Determinants of Health (e.g. MLP, CSS, Food Pantry)
- Support the work of the Prevention Case Manager; assist in care coordination and follow-up
- Demonstrate knowledge of and compliance with regulatory agencies and established organizational policies and procedures
- Schedule patients for follow-up appointments based on established protocol, identified need and availability of appointments;
follow-up with patients who have to keep important appointments
- Exhibit customer service consistent with organizational standards
- Openly, clearly and respectfully share and receive information, opinions and concerns and feedback in a supportive manner
- Work collaboratively by mentoring new and existing co-workers, building bridges and creating rapport with team members across the organization
- Other duties as assigned
EDUCATION AND EXPERIENCE :
- Minimum of High School Diploma or GED
- Bachelor’s degree in health or related field preferred
- 2 years professional experience in a health or behavioral health practice preferred
