Job Summary
The Social Worker, BSW is responsible for assisting patients with understanding how to utilize health care and social welfare resources so to obtain optimal care.

Job Specific Responsibilities
Daily assignments may include but are not limited to :
- Provide leadership for Radiation Therapy and Dosimetry staff
- Assess the patient's ability to participate actively in their medical care post discharge
- Educate patients / family and refer them to the appropriate community resources
- Remain up to date on resources available for patients / families.
- Facilitate referrals and transfers to nursing homes, skilled nursing facilities, rehab, and long-term acute care facilities.
Coordinate for the appropriate mode of transportation (ambulance, wheelchair van ) as needed by the patient for the transfer.
- Arrange for durable medical equipment, out-patient services, Home Health and / or Hospice care as needed.
- Complete high-risk screens and length of stays screens on unit in accordance with DPP# 102.
- Participate in huddles on unit and other huddles as requested to identify needs or anticipated needs for the patient at discharge.
- Documentation in Allscripts / Cerner in a complete and timely.
- All other related assigned duties
Education and Experience
Bachelor’s degree in Social Services or related field
Required Licensures / Certifications / Registrations
LBSW / LMSW- Licensed Baccalaureate or Licensed Master Social Worker licensed in the state of Texas by the Texas Board of Social Work Examiners *Active temporary license issued by the Texas State Board is acceptable if permanent license is obtained prior to temporary license expiration.
Proof of passing examination score provided by Association of Social Work Board testing site

- Excellent communication skills, both verbally and in writing; Ability to communicate with empathy and influence
Interaction with Other Departments and Other Relationships
This position requires interaction with medical staff and Allied Health partners
Physical Capabilities
Visual and auditory acuity required. Position requires much walking and standing for extended periods of time
Environmental / Working Conditions
Position subject to inside environmental changes that may include varying changes in temperatures.
