If you are pursuing a career in the social services field and have an interest in working in behavioral health, our Discharge Planner position would be a valuable experience.
The discharge planner is responsible for assisting in discharge planning coordination with unit, social worker and Utilization Review staff.
This position entails attending treatment planning meetings, reviewing clinical documentation and seeking appropriate aftercare referrals.

Qualifications
Education : High school graduate, bachelors preferred. Completion of basic clerical or medical / clerical course is preferred.
Experience : A minimum of two (2) years’ experience in a clerical support role, preferably in a psychiatric health-care setting.
