The Therapy Case Manager, manages all aspects of accepting and managing all therapy referrals, the field therapist’s schedule based on a patient’s plan of care, daily patient referrals as well as manage the schedule for existing patient schedules.
This is a temporary position with potential to extend into a permanent one.
DUTIES AND RESPONSIBILITIES :
- Schedules field therapists appropriately to patients in accordance with the patient’s plan of care.
- Assists with new employee orientation
- Coordinates all therapy for the patient including scheduling patient visits and coordinating services.
- Reviews and understands the admission process for the plan of care of each patient.
- Coordinates plan of care with therapy providers.
- Reviews therapy evaluations and visit notes.
- Follows Medicare regulations.
- Maintains a good working relationship with all contracted companies.
- Assists with additional scheduling responsibilities as needed.
QUALIFICATIONS : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.

- Education and / or Experience High school Diploma required; 1 year of Home Health Care experience preferred.
- Language Skills Ability to read, analyze, and interpret common patient information, medical documentation and insurance information.
Ability to respond to common inquiries or complaints from clients and therapy providers. Ability to effectively present information to management and co-workers.
Ability to interact clearly and effectively, in both written and oral communication, with supervisor, clients, staff, and providers, etc.
Computer Skills - To perform this job successfully, an individual should be proficient in personal computer skills including electronic mail, record keeping, routine database activity, word processing, spreadsheet, etc.
Ability to navigate through specific company software used for patient information.
- Other Qualifications - Strong organizational Skills and understanding of information storage and retrieval required.
