Position Summary :

Under the supervision of an RN, performs basic patient care activities. Records all entries, vital signs, and data using the appropriate manual forms or electronic charting.

Create and maintain a calm atmosphere throughout the unit and through the shift. Monitors for any resident change of condition and promptly reports to the nurse.

Assists with performing tasks that adhere to residents' plan of care and choice in resident-centered environment.

Reports to : Director of Nursing

FLSA : Non-exempt

Qualifications

Minimum Qualifications :

Minimum Education

x High school diploma or equivalent preferred. Required Skills

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Required Skills

Certifications :

x Basic Life Support (BLS) AND First Aid OR First Aid CPR AED within 30 days of hire

x Food Handlers Certification within 30 days of hire

XFingerprint Clearance Card underway on hire, Card must be turned in within 60 days after hire Required Skills

x AZ Nursing Assistant Certification required on hire
