The Outreach and Rapid Response Case Manager will work on the streets in a virtual mobile office focusing primarily on chronically homeless individuals, including those that were recently or are currently incarcerated.
The Case Manager will primarily target, but not exclusively, the downtown area, encampments, and homeless gathering areas and will work in continuous collaboration with friends and family members, shelters, outreach / case managers, law enforcement, fire department, ambulance services, business owners / operators, faith communities and ordinary citizens.
The Case Manager will collaborate closely with other service systems, help identify individuals who are not currently being engaged by other care systems, work collaboratively with individuals, businesses, and systems that are regularly engaged with people living on the streets, gather critical information regarding their needs, and helping to formulate plans and interventions designed to help people secure permanent housing and obtain needed resources or services such as legal, financial, medical, social / recreational, etc.
PRINCIPAL DUTIES AND RESPONSIBILITIES :
- The Outreach and Rapid Response Case Manager builds relationships with some of the most difficult to help and serve chronically homeless individuals not already engaged by other care providers.
- The Case Manager will communicate and collaborate with all other engaged systems, public and private, to assist in any way necessary to secure needed housing and services.
- The Case Manager will collaborate and help convene regularly scheduled and ongoing, multi-organizational staffing to help create collective impact interventions aimed at helping people secure housing and services required for the individual, connect individuals to other needed community services, connecting to family and friends whenever possible, making sure the individual is on the by-name list when appropriate, and providing warm handoff to other care systems to ensure ongoing housing and care stability.
KNOWLEDGE, SKILLS & ABILITIES :
- Must be in active recovery for a minimum of 2 years.
- The Outreach and Rapid Response Case Manager will have a minimum of a bachelor's degree in a human relations / behavioral health / medical health related field and 1-year experience working with homeless populations OR 3+ years working extensively with homeless populations.
- The individual must possess outstanding communication skills and interpersonal comfort with people who may appear aggressive at times, people with histories of varying types of severe and chronic illness, personal hygiene issues, and untreated mental illness.
- The individual must demonstrate a clinical skill set in the area of crisis intervention, crisis management, trauma informed care, harm reduction, and de-escalation as there will inevitably be situations that will require the aforementioned areas of expertise.
- Demonstrated experience in critical thinking and problem-solving skills will also be necessary.
WORK CONDITIONS & PHYSICAL DEMANDS :
Employees are required to : walk and climb stairs; reach with hands and arms; be mobile by moving oneself from place to place quickly and easily;
carry and lift; climb hills; bend, stoop or kneel.
- Employees will be subjected to outside environmental conditions while conducting various levels of outreach.
- Employees will be required to work in areas inhabited by homeless people, to include working in dirty environments and dealing with challenging individuals.
- Ability to effectively communicate orally and in writing.
- Physical ability to lift and carry 50 pounds.
- Visual and auditory ability to identify and respond to environmental and other hazards of the site and staff behavior.
- Physical mobility / endurance to perform tasks while standing / walking for long periods of time (60 minutes or more).
- Ability to safely and properly use office equipment.
- Potential exposure to stressful and / or physically challenging situations that could result in risk to one’s safety.
