TSINY is seeking a Medical Coordinator for one of our residential location in Queens Village (Queens), NY
Medical Coordinator - Full-Time
Salary : $50,000 annually, $27.47 Per Hour
License Required, applicants must have Clean Driving Record
Position Summary
Responsible for coordinating and monitoring the medical and dental care for all residents. Serve as an advocate and intermediary for and on behalf of residents’ rights to quality care.
Work in partnership with our residents and other program staff to ensure the continuity of appropriate care.
Essential Functions
- Ensure that residents receive required routine and emergency medical and dental care.
- Provide assistance to schedule appointments.
- Escort individuals to appointments.
- Monitor follow up to appointments.
- Ensure care is appropriately documented according to TSINY standards.
- Ensure residents have an adequate supply of medication and medication supplies at all times.
- Complete TSINY #123 forms weekly.
- Facilitate Medication Management Training Groups with residents.
- Complete and document monthly medication reconciliations.
- Maintain contact with all related treatment providers to insure integrated care between the residential program and other services.
- Meet on a regular basis with interdisciplinary team for assessment and information on resident.
- Visit hospitalized clients as necessary.
- Monitor medication when necessary.
- Provide crisis management when required.
- Work with support staff to coordinate services and discuss outstanding clinical issues.
- Be able to sit or stand as needed, with or without reasonable accommodation.
- May require walking, primarily on a level surface, for short periods throughout the day, with or without reasonable accommodation.
- Be able to reach above shoulder heights, below the waist or lift as required to file documents or store materials throughout the workday, with or without reasonable accommodation.
- Driver’s license required.
- During a declared disaster, assume and adhere to assigned Job Action role(s) consistent with Disaster Preparedness Plan.
- Perform other related duties as required.
Experience / Education / Skills / Abilities
Bachelor’s degree or Associate’s degree plus one year’s experience or High School diploma plus two years’ experience in a mental health setting.

- Have the ability to interact effectively with all levels of employee, as well as external contacts.
- Be able to handle confidential and sensitive information.
- Be proficient with basic office equipment such as telephones, copiers, fax machines, printers, PCs.
- Be able to work effectively using program management and other tools on a personal computer for long periods of time.
- Possess the ability to read and interpret documents, such as safety rules, operating and maintenance instructions and procedure manuals.
- Be able to write routine reports and correspondence.
- Be able to define problems, collect data, establish facts, draw valid conclusions and evaluate alternative courses of action.
- Be able to effectively present information and respond to questions from groups of managers and clients.
