Overview :
Are you skilled at coordinating and making connections? We are seeking a Clinical Liaison Coordinator to facilitate patient referrals, build relationships with hospitals, and educate the community on hospice care.
In this role, you will work closely with patients and their care team to ensure continuity of care and proper documentation.

Qualifications :
- A heart to serve patients and families
- Education : Minimum of an Associate Degree in a health-related field or equivalent work experience
- Experience : 2 years of sales experience in a clinical care setting, hospice preferred
- Requirement : Reliable transportation and ability to lift at least 25 lbs
