Scope of Position
The Health Behavior Counselor is a direct patient care position addressing aspects of substance dependence utilizing a patient-centered harm-reduction approach.
The majority of work tasks are complex; independent judgment and skills are required. Supervision is required for some decisions in the day-to-day decision making related to patient care.
Engages in screening and initial assessment of substance use disorders, and provision of individual alcohol and other drug (AOD) interventions, harm reduction, and health promotion interventions;
these are patient-centered harm-reduction interventions to help initiate and maintain positive behavior change, and to empower patients with the information necessary to make informed decisions about their health and well-being.

Position Summary
The counselor is responsible for facilitation of an individualized supportive care plan related to the risk of chemical dependency in all levels and locations of a patient's care.
The position is responsible for the creation and implementation of the treatment plan. The position requires special knowledge, skills, and competencies relating to chemical dependency / mental health, education and treatment in conjunction with cancer care and pain management.
Minimum Qualifications
Master's Degree in Social Work or Counseling; Ohio License to practice (LSW or LPC); independent license preferred (LISW or LPCC).
