EMPLOYMENT SPECIALIST / JOB COACH
MISSION STATEMENT :

Immediate Supervisor : Community Services Manager
Work Schedule : Hourly Status-Work schedule may vary; may include evenings and weekends.
Overall Responsibilities : To provide on the job training assistance, counseling, and evaluation of a newly placed individual(s) in a community work setting, transport individuals to and from work sites, as needed.
Minimum Requirements : High school diploma or equivalent and a minimum of one (1) year experience working in a rehabilitation facility or other related human / social services field, Valid Public Passenger Chauffeur's License, CPR and First Aid Certification, subject to random drug testing.

Primary Responsibilities : Responsibilities will include, but not limited to the following :
1. Schedule meetings with new consumers to develop a personal resume, prepare consumer for interviewing, and assist consumers in filling out job applications
2. Assist with community site interviews and job orientation with consumers, as needed
3. Job training and onsite job coaching for tier 1 consumers at the community sites for 90 days or more, if needed
4. Job training and onsite job coaching for tier 2 consumers at the community sites for 90 days
5. Assist community site management and / or consumer with any necessary employment issues
6. Prepare twice a month reports on each consumer in Follow Along Support Services, as required by DDARS and / or Waiver services
7. Complete and maintain all paperwork for each consumer, as required by Vocational Rehabilitation Services for the State of Indiana
8. Responsible for documenting all received training on appropriate form and submit to Human Resources on a monthly basis
9. Arrive on time, ready to work, as scheduled and depart from work on time, as scheduled
10. Attend workshops, in-service training, conferences and meetings as assigned
11. Comply with all agency policies and procedures
12. Responsible for attending the agency's strategic planning process and meeting goals of the agency's strategic plan as applied to the Department
13. Accepts other duties as assigned by the Director of Community Services of Janus consistent with the agency mission and goals.
Physical Requirements :
- Sitting in a normal position for extended periods of time
- Hand-eye coordination necessary in order to operate computers and various office equipment
- Ability of lifting and moving packages weighing up to 25 lbs
- Must be free from fainting spells, blackouts, dizziness, or seizures
