Job Summary
Serves as a point of contact for referring physicians, patients, and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within the Hospital and within in the community.
Facilitates patient appointments, including those made with labs, diagnostic areas, and specialty physicians to ensure an appropriate continuum of care.
Minimum Job Requirements :
Work Experience
Three years of experience as a Registered Nurse required. Previous case management experience or related experience preferred.
License / Registration / Certification :
Current TX / Compact RN License required.
Current CPR required.
Certified Breast Care Nurse (CBCN) preferred.
Education and Training :
Bachelor’s degree in Nursing required; Master’s degree preferred.

- Organize, direct, coordinate and evaluate specialized diagnostic breast imaging to ensure quality patient diagnosis and care that conforms with prescribed standards and regulations
- Analyze service delivery problems accurately, develop recommendations and take appropriate action to resolve them
- Develop, evaluate, and maintain accurate record-keeping systems, prepare complex and detailed written reports, procedures, and contracts
- Communicate effectively with administrative, medical, professional, and clinical staff and citizen groups
- Work cooperatively with other University Medical Center and County departments, public and private organizations, including public and private medical providers
- Train staff and coordinate activities of contractors, volunteer groups and staff, as appropriate
- Work tactfully, sympathetically, and effectively with various typed and conditions of patients
- Demonstrated use of clinical skills and expertise in the area of breast disease, breast cancer treatments and psychosocial impact of disease on recovery
- Ability to comprehensively assess patient’s physical and psychosocial status
- Ability to effectively and efficiently coordinate, implement and monitor the care management process and treatment plan for each patient
- Ability to communicate openly and empathetically with patients and families
- Ability to work collaboratively with multidisciplinary care team to develop and implement care plan
- Ability to develop and implement in-house (professional development) and community education outreach programs / workshops / seminars / support services
- Ability to communicate effectively with diverse audiences including professional staff, patients, community, media, and other service agencies
- Knowledge of adult learning competencies and patient teaching skills
- Demonstrated management skills reflecting self-motivation, initiative and fiscal responsibility
- Tolerance of ambiguity and the flexibility to respond to changing system needs
