Summary
The Case Management Manager assists the Director of Integrated Care Management (ICM) in leadership, coordination, and supervision of the case management department.
This position is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources while respecting patient's right to self-determination.
The Manager has the overall responsibility for ensuring care is provided at the right level, right time and with the right resources.
Primary duties of this position include supervision of day-to-day activities, financial accountability, authority to hire, and providing coaching for performance improvement to staff as needed in collaboration with the Director.
Responsibilities
- Assisting staff in care coordination by demonstrating efficient throughput while assuring care is sequenced and at the appropriate level of care
- Generate reports and provide data analysis on metrics, outcomes, and trends to support performance improvement initiatives and compliance with department and CTH guidelines
- Examine patient medical records to ensure the necessity and appropriateness of care provided
- Work closely with other healthcare professionals, patients, and payers to assess, plan, coordinate, monitor, and evaluate the delivery of healthcare services and appropriate levels of care
- Shares accountability with other medical center leadership for the daily identification and escalation of problems, and initiation and evaluation of action plans for achieving medical center targets and improve the quality of care and services
- Collaborates with interdisciplinary teams across the continuum of care including, but not limited to Nursing, MSW, PT / OT, HH, Hospice, SNF, Behavioral Health, Rehabilitation, etc.
to ensure patient care is effectively provided, clinically appropriate, service oriented, safe and cost effective
- Document patient-related data in a clear and organized manner using standardized templates, capturing essential details such as patient assessments, medical necessity, collaborative communications, and relevant outside agency interactions
- Monitors daily workflow and adjusts assignments to assure optimum workflow
- Effective collaboration with staff, physicians, nurses, ancillary staff, payors, patients and families to achieve optimum clinical outcomes
- Maintains productivity by assuring staff are completing appropriate roles functions and assignments are logical
- Assists staff in care coordination and demonstrating efficient throughput while assuring care is sequenced and at the appropriate level of care
- Promotes effective communication through verbal, written, and listening skills
- Anticipates needs, problems or pending crisis and develops action plan to resolve
- Continuously demonstrates a positive attitude and understanding of case management philosophy, supports team building, and is motivated to fulfill department objectives
- Maintains current knowledge of disease processes, available resources, and treatment options appropriate to patient population
- Manages and resolves human resource, employee, department safety, and risk management issues. Responsible for all aspects of staff management including, hiring, development / training, performance reviews and terminations
- Participation in department quality improvement initiatives and projects as assigned
- Promotes current National Standards for Case Management and relevant clinical / case management practices
- Performs other related duties as assigned
Qualifications
Required : o Minimum two (2) years of acute care hospital patient care experienceo Two (2) years progressive leadership experienceo Current unrestricted Register Nurse license in the State of Nevadao Understanding of Federal Regulations related to discharge planning and Hospital utilization managemento Organizational skills, excellent verbal & written communication skills, ability to lead, coordinate diverse group in fast paced environment, critical thinking, problem solving and negotiation skillsPreferred : o Bachelor of Science in Nursingo Accredited Case Manager (ACM)o Minimum two (2) years of experience as acute care case manager
