Position Summary :
The Licensed Practical Nurse (LPN) PRN position at Facility Name is responsible for providing direct patient care under the supervision of a Registered Nurse (RN)
The LPN will assist in maintaining a safe and therapeutic environment for patients.
Essential Duties and Responsibilities :
- Patient Care :
- Administer medications as ordered.
- Perform treatments and procedures as directed.
- Assist with patient activities of daily living (ADLs).
- Monitor vital signs and report any changes to the RN.
- Observe patients for signs of distress or deterioration and report findings to the RN.
- Documentation :
- Accurately document patient care activities in the medical record.
- Complete incident reports as necessary.
- Communication :
- Communicate effectively with patients, families, and other healthcare team members.
- Participate in patient care planning and coordination.
- Quality Improvement :
- Assist in implementing quality improvement initiatives.
- Adhere to facility policies and procedures.
Qualifications :
- Current LPN license in good standing.
- Experience in a skilled nursing facility preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Excellent organizational and time management skills.
