SUMMARY Assists individuals and families requiring assistance by performing the following duties.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
- Assess the need and eligibility of referred elderly and people with disabilities for governmental and nongovernmental services which include home and community-based services and nursing home placement and provides linkage to community services as appropriate.
- Use independent discretion and judgment in developing intervention strategies and comprehensive plan of care with clearly stated goals and objectives in conjunction with client, family, or others.
- Review and documents service plan quarterly.
- Perform follow-up to determine quantity and quality of service provided and status of client's case.
- Act as an advocate to preserve client rights and maintain client confidentiality.
- Maintain up-to-date knowledge of services, programs and intervention strategies affecting the elderly and people with disabilities.
- Meet and maintain annual certification requirements of the FSSA Division on Aging.
- Document all client interactions and service linkage activities within prescribed time frames.
- Access and record client and community resource information.
- Analyze and review the eligibility of clients for referral to community resources and other organizations.
- Compile, record, and prepare reports within prescribed time frames.
- Analyze and review the eligibility of clients for assistance.
- Represent CICOA at public functions as requested.
- Travel locally as needed to complete client assessment.
- Perform other duties as assigned.
SUPERVISORY RESPONSIBILITIES
This job has no supervisory responsibilities.
QUALIFICATIONS

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Currently, the Case Manager position does NOT require licensure of any kind but does require for the staff member to obtain Case Management certification through the Division of Aging.
This certification is part of the orientation process of the position.
EDUCATION and / or EXPERIENCE
Bachelor’s and / or Master’s degree from accredited college or university in social work, gerontology, counseling, psychology, or nursing;
or a RN Licensure with one year experience in Human Services; or Bachelor’s degree in any field with a minimum of 2 years full time, direct service experience with the elderly or disabled which includes assessment, care plan development and monitoring.
