Job Summary :
Primary functions are to carrying out clinical examination of patients in their care, performing diagnosis based on their assessment of the patient, coordinate care with the interdisciplinary group.
Responsibilities :
- Under the supervision of a physician, the nurse practitioner visits patients, completes physical assessments, performs face-to-face assessments and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team.
- Provides medical care, palliates, and manages terminal illness in compliance with the state Nurse Practice Act.
- Completes history and physical examinations, by evaluating the patient’s medical condition and
- Manages health conditions and terminal illnesses.
- Completes the hospice face to face visit requirements.
- Provides clinical information to the hospice medical director for interpretation for eligibility for hospice recertification.
Job Qualifications :
Education / Licensure : Required to be a licensed Nurse Practitioner with the State Nurse Practice Act Licensed in the state of operations where assigned branch is located.
Experience :
- Minimum of one year of Nurse Practitioner experience required.
- Prefer a minimum of six months to one year of experience in hospice or pain and symptom management.
Skills :
- Nursing skills as defined as generally accepted standards of practice.
- Excellent interpersonal skills and the ability to communicate effectively with all segments of the staff.
- Demonstrates proven decision-making skills.
- Proof of current CPR.
Transportation :
- Reliable transportation.
- Valid and current auto liability insurance.
- Environmental and Working Conditions : Works in patients' homes in various conditions, possible exposure to blood and bodily fluids and infectious diseases;
ability to work a flexible schedule; ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
Physical and Mental Effort : Prolonged standing and walking are required, with the ability to lift up to 50 lbs and move patients.
Requires working under some stressful conditions to meet deadlines and patient needs, and to make quick decisions and resource acquisition;
meet patient / family individualized psycho-social needs
