Social Services Director
Facility : Palm Beach Nursing and Care Center
Salary : up to $68,000 based onn experiance
Are you passionate about making a difference in the lives of residents and their families? Palm Beach Nursing and Care Center is seeking a dynamic Social Services Director to lead, organize, and manage the Social Services Department, ensuring our residents feel supported and empowered during their stay.
Responsibilities :
- Oversee patient assessments, care planning, and help residents and families smoothly transition into their new environment.
- Lead the development of policies and goals for the Social Services Department to enhance resident care and satisfaction.
- Collaborate with various departments to resolve escalated issues and ensure smooth operations.
- Regularly assess patient needs to ensure the facility continues to meet them, providing families with relevant care plan updates.
- Engage residents and families in the planning of social service programs to enhance their experience.
- Provide practical solutions for social and environmental challenges, including financial assistance, discharge planning, and referrals to specialized community resources.
- Actively participate in facility surveys and inspections by authorized government agencies.
- Interpret and communicate the social, psychological, and emotional needs of residents and their families to the care team, medical staff, and attending physicians.
Requirements :
- Minimum of 3 years of experience in a Skilled Nursing Facility.
