This individual will provide crisis intervention for individuals known to the agency, conducting follow-up visits with existing clients and with other individuals based on referrals from law enforcement.
As appropriate, this Specialist will link individuals with appropriate mental health services and other community supports.
What You'll Do : Conduct assertive follow-up of individuals on Assisted Outpatient Treatment.Coordinate with case management and other providers as appropriate.
Conduct coordinated wellness visits with law enforcement at the request of agency staff.Assist in completion of emergency admission to Psychiatric Emergency Services or hospital.
Incorporate the principles of trauma informed care into practice, recognizing how trauma may influence health practices.Coordinate and present portions of the ADM sponsored CIT training held at Community Support Services several times per year.
Complete and submit documentation in a timely manner, including, but not limited to, progress notes and treatment plans.What We're Looking For : Minimum of a Bachelor's degree in a human services field required.
Minimum of LSW required.A minimum of 2 years' experience in case management with persons with severe and persistent mental illnesses.
Knowledge of local community resources and a demonstrated ability to access.A car and a valid Ohio driver's license and insurability through the Agency's insurance carrier required.

Ability to develop positive relationships with individuals with mental illness, developmental disabilities, and substance abuse challenges.
