Nurse-Family Partnership (NFP), a maternal and early childhood home visiting program, fosters long-term success for first-time parents, their babies, and seeks to address some of the serious systemic problems affecting our community.

Building on this research, the NFP Community Health Worker addresses the fragmented structure of the social services industry by taking a holistic, whole-family approach.

This position will report to the Manager, Nurse-Family Partnership Community Health Workers.

The NFP Community Health Worker (CHW) Intern will coordinate solutions to assist Indiana NFP teams, particularly serving rural areas, with directing their participating families to quality resources in their communities based on each family's specific needs, strengths and motivation as identified by their Nurse Home Visitor(s) and theManager, Nurse-Family Partnership Community Health Workers.

This position will be for 120 total hours and pay $12.50 per hour plus mileage for travel(s).

Example Duties and Activities

Assists with building relationships with NFP staff, NFP families, and networks with key community partners including Goodwill staff (community agencies, educational institutions, financial liaisons, and employers) to enhance the menu of wrap-around services provided

Assists with creating easily accessible rapid-response and long-term solutions for NFP nurses and families to connect effectively to quality community resource networks

Assists with ensuring timely and accurate entry of required information into NFP databases and files.

Participates as needed in NFP CHW team meetings

Participates in continuous innovation projects that improve operational efficiency and evidence-based outcomes

Required Competencies

Completion (or pending certification) of Certified Community Health Worker (CCHW)

Technical Knowledge - Has working knowledge of Google Suite.

Self- and Team-Reflection - Evaluates one's own attitudes and actions to recognize successes and areas for self-improvement.

Geographical Proficiency - Has extensive knowledge of Indiana's rural communities.

Completes all assigned reports. Reads, interprets, and follows financial reports to be able to communicate information to others and translate data to other communications.

Other Requirements

Has the ability to occasionally work a flexible schedule beyond normal business hours.

Is willing to travel to participants' homes, workplaces, community centers, and other venues at times that are convenient to individuals being served.

Background Screening - All candidates must pass a pre-employment background check. Must have a valid driver's license, up-to-date auto insurance, and a good driving record.
