What's your day like?
As a new Memory Care Coordinator at The Heritage of Twin Creeks Assisted Living, you'll be responsible for creating and implementing personalized care plans for residents with memory impairments.
Your day-to-day tasks will include conducting assessments, coordinating with healthcare providers, leading care staff, and providing emotional support to residents and their families.
You'll also facilitate engaging activities tailored to the unique needs of each individual, ensuring a safe and nurturing environment within our community.
Your role will require excellent communication skills, attention to detail, and the ability to handle challenging situations with empathy and professionalism.
Requirements for this Memory Care Coordinator job
To excel as a Memory Care Coordinator at The Heritage of Twin Creeks Assisted Living, candidates must meet the following qualifications :
- Possess at least a high school diploma or equivalent.
- A minimum of two years of prior senior living experience, preferably with at least one year of prior Caregiver and / or Med Tech experience.
- Candidates should be at least 18 years old, have basic computer skills, and be proficient in speaking, reading, and writing in English.
These skills are essential for fulfilling the responsibilities of creating personalized care plans, coordinating with healthcare providers, leading care staff, and providing emotional support to residents and families on a daily basis.
